A onetime form that covers all London youth events. The form is detailed, but once filled in you don’t need to repeat this form for the next event unless something changes. We will use the information supplied for future events organised through the London CYDW (Julia Stacey), which your young person participates in, so please keep us informed if any of the information on this form changes.
