Offer of service form
In order to complete this form, you will need:
– 10-20 minutes
– Some thoughts about your experience with children and young people and why you want to volunteer
– Details of two people we can contact to provide a reference (one Quaker and one non-Quaker)
– If you hold a First Aid qualification, the name of provider and expiry date
– If you have a valid DBS check (less than 3 years old at the start of the event), a photo or scan you can upload.
The deadline for completing this form is Monday 1 December 2025.

Applying for a DBS
Once we have made a decision about whether to take up your offer of service, we will initiate the online application with some basic information from this form. You’ll receive details from DDC and ourselves about how to proceed.

DBS Update service
This is an online service that allows your records to be checked by you, your employer or whoever you authorise to. If you are not sure whether you have joined this or not, you can find out at: www.gov.uk/dbs-update-service.

We will need to see your original DBS certificate. When you show this to us, we will need to take a photo or photocopy for our records.
Allowing BYM to use the DBS Update Service and providing an copy of your DBS Certificate for our records will mean that you are unlikely to need to obtain a new DBS check.

References
Please provide the details of two people (not related to you) to whom reference can be made. Both should know you well and at least one should have known you for two years or more. It is fine if one of these referees is a young person or child. If possible at least one of your referees should be able to comment on your experience of, attitudes towards and behaviour with children and young people. One should be a Quaker and one should know you from another area of life. We will only contact these referees if the references we have for you are more than 3 years old.
